In December 2016 Hope was funded by the WA Primary Health Alliance (WAPHA) to provide the Integrated Team Care (ITC) program in the Goldfields.  The program aims to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care and to contribute to closing the gap in life expectancy by improved access to culturally appropriate mainstream primary health services (including but not limited to general practice, allied health and specialist) for Aboriginal and Torres Strait Islander people.

The program supports Aboriginal people with chronic health conditions to access mainstream healthcare and also provides funding for supplementary services and medical aids. Services are provided in Leonora and Laverton in a sub contract arrangement with Pier Street Medical through a Care Coordinator and Aboriginal Outreach Worker.  Services are also provided In Kalgoorlie through a Care Coordinator, Aboriginal Outreach Worker and Indigenous Health Project Officer and in Esperance through a Care Coordinator and Aboriginal Outreach Worker.
