Integrated Team Care  (ITC)

HOPE provides support for Aboriginal people in the Goldfields who have chronic health conditions, so they can access mainstream healthcare services and access funding for supplementary services and medical aids.

WAPHA funds HOPE to provide the Integrated Team Care (ITC) program in the Goldfields to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions, through better access to coordinated and multidisciplinary care.

ITC also contributes to closing the gap in life expectancy by improving access to culturally appropriate mainstream primary health services (including general practice, allied health and specialist) for Aboriginal and Torres Strait Islander people.

A Care Coordinator and Aboriginal Outreach Worker provide the service in Leonora and Laverton under contract with Pier Street Medical. In Kalgoorlie, a Care Coordinator, Aboriginal Outreach Worker and Indigenous Health Project Officer deliver the service and in Esperance we have a Care Coordinator and Aboriginal Outreach Worker.
