Psychosocial Support Coordination Program

Coordinating support, fostering recovery and independence.

Moorditj Koort Aboriginal Corporation and Hope Community Services are now working in partnership to coordinate the following programs and services:

  • Individual Community Living Strategy (ICLS)
  • SOAR (Support, Opportunities And Resilience for Youth)
  • SOAR Plus (YTH&SP).

Previously managed by the Mental Health Commission, the day-to-day coordination of these program and packages has been entrusted to Moorditj Koort and HOPE as of April 2026.

Together, our goal is to provide clear and equitable coordination of psychosocial support programs and services that empower vulnerable young people and individuals to access the accommodation, services and care they need as they work towards recovery and independence.

Our experienced and knowledgeable Program Coordination Service (PCS) team is now in place and looking forward to working with support providers to deliver the best outcomes for clients.

Psychosocial support programs

The three programs within our coordination remit are focused on building clients’ social, emotional and life skills.

Please note, HOPE does not provide these programs directly. If you are seeking support for yourself or another, please contact PCS Team so they can put you in touch with a local service provider.

SOAR Packages

The SOAR program assists young people aged 16-24 years, with moderate to severe mental health issues (with or without co-occurring alcohol and other drug challenges) with short-term support to manage daily life.

SOAR Plus Packages

The SOAR Plus program assists young people aged 16-24 years, with moderate to severe mental health issues to live independently and work towards their recovery goals.
